A license and a privilege sit in different places in the structure, and the difference decides what happens when something goes wrong.
A license stands on its own
A state board issues it after examining your credentials directly: certification, education, license history, background check, and whatever supervision or collaboration arrangement that state requires. It is valid in that state and nowhere else. If your license in another state lapses, this one is unaffected.
A privilege rests on a license
A compact privilege authorizes you to practice in a member state on the strength of an unencumbered license you hold in a different member state. The issuing state has not examined you; it has agreed to rely on the state that did.
The consequence follows directly. If the underlying license lapses, is suspended, or is disciplined, every privilege built on it falls at the same moment. Not sequentially, not after a review — they are derivative, and they go together. A PA holding one license and five privileges has one point of failure, not six independent authorizations.
Why this matters before privileges even exist
It changes which license you want to be holding. If your qualifying license is going to carry your ability to work in several states, its renewal date, its CME requirement and its good standing stop being routine administration and become the thing your whole practice depends on.
It also changes the maths on letting a license lapse. A second license in a member state looks redundant while you are only working in one place. It stops looking redundant the moment your primary license is the only thing holding up everything else.
