Career Breaks: Restoring Certification After Time Away

Coming back is a sequence, not a single step, and the order is fixed. Certification first, then licenses, then everything that depends on them.

Career breaks are ordinary — parental leave, caregiving, illness, a spell in a non-clinical role, military service. Returning is a well-worn path, but it has an order that cannot be rearranged.

The order

Certification first. Every license application requires it, so nothing downstream can start until it is current. If it lapsed, restoration has its own requirements, potentially including examination.

Licenses second. Depending on how long the gap ran, this is either a renewal, a reinstatement, or a fresh application. Different states put the cliff in different places.

Everything else third. DEA registration, state controlled-substance registration, collaboration agreements, payer enrollment. Each has its own lead time and several cannot begin without a license number.

Why the sequence hurts

Because it is genuinely sequential. Someone planning a return in three months, working from the assumption that these run in parallel, is usually looking at something closer to six.

The gap question

Boards frequently ask what you were doing. This is not a trap — it is a routine question with a routine answer. Answer it plainly and factually. Applicants get into difficulty by being vague, which invites follow-up, not by having taken time away.
